Diabetes self-management education is a collaborative process through which people with diabetes gain the knowledge and skills needed to modify behavior and successfully self-manage the disease and its related conditions

ESPERANZA Y VITA
Trained Lay Health Advisors, Witnes Role Models and Peer Navigators educate, support and help navigate cancer patients through the healtcare system, helping to connect patients with resources and clarify myths and facts surrounding cancers.

QUINN COMMUNITY OUTREACH CORPORATION Works diligently to provide you with quality, fresh food supplies. Serving over 500 individuals each month, we aim to supplement family grocery budgets and help those in need.Each month, the community is welcome to come for pick-up, and we also offer limited delivery options to ensure everyone receives the support they need.
