  • A Free Service to Participants.

  • Classes Held Locally in the Community.

  • Delivered through culturally relevant interactive sessions rananging from 10 - 12 hours of instructions.

  • Available in English and Spanish.

  •  Designed to follow National Standards for indivisualized education and support for participants.

  • Focused on helping particpants achieve their personal diabetes and self-management goals by delivering content that addresses:

    • Understanding the Human Body as Related to Diabetes.

    • Understanding Risk Factors for Diabetes.

    • Monitoring Your Body and Using the Results for Self-Management and Decision Making.

    • Get up and Move Diabetes & Physical Activity.

    • Managing Diabetes Through Nutrition and Meal Planning.

    • Diabetes Complications: Identification and Prevention.

    • Medications and Medical Care

    • Personal Strategies to Manage Psychosocial Issues and Promote Behavior Change, by Mobilizing Family and Friends
